Our Collaborative Care Services

Collaborative care management (CoCM)

is a patient-centered approach that involves the coordination of healthcare providers from different disciplines to address the complex needs of individuals with substance use disorders. This approach typically consists of a team of professionals, including primary care providers, addiction specialists, mental health professionals, and social workers, working together to develop a personalized care plan for the patient.

Cognitive-behavioral therapy (CBT)

Has been widely recognized as the gold standard for the treatment of substance use disorder. CBT is an evidence-based approach that helps individuals identify and change negative patterns of thinking and behavior that contribute to substance use. It also teaches individuals effective coping strategies, stress management techniques, and problem-solving skills to manage triggers and cravings associated with substance use.

Contingency management (CM)

Is a highly effective approach in the treatment of substance use disorder. CM is a behavioral therapy that uses positive reinforcement to encourage individuals to remain abstinent from drugs or alcohol. Just like with anything in life, the more positive reinforcement you have when attempting something new, the better your results will be. CM is a valuable tool in the treatment of substance use disorder and helps maintain a meaningful recovery.

Problem-solving therapy (PST)

Is a technique associated with helping people make better decisions about their health and lifestyle that has shown promising results in helping individuals with substance use disorder. PST is a structured, evidence-based intervention that focuses on breaking down behavioral changes into incremental and actionable steps that teach healthy decisions. These behavioral changes are mostly associated with SDOH and lifestyle decisions.

Remote therapeutic monitoring (RTM)

Is a cutting-edge approach to helping individuals struggling with behavioral health issues and substance use disorder. By utilizing technology such as wearable devices, mobile apps, and virtual counseling sessions, remote therapeutic monitoring allows for continuous monitoring of an individual’s physical and mental health. RTM allows therapists to help patients on their terms and from a location that is most comfortable for the patient.
