Holon Health, Inc. and its affiliates (collectively, “Holon”) are committed to providing care that respects your dignity, your decisions, and your rights as a patient. This Patient Rights statement describes the rights you have when you receive care from Holon. It is a public-facing summary of the rights set forth in your Client Agreement and in our Notice of Privacy Practices (NPP), and it applies to all Holon patients in every state where Holon operates.

Your Rights
Respect and dignity
You have the right to be treated with respect and dignity. You have the right to have your cultural and religious values respected by your care team. You have the right to receive care that is safe and free from abuse or neglect.
Non-discrimination
Holon does not discriminate on the basis of race, color, national origin, age, disability, sex (including pregnancy, sexual orientation, and gender identity), religion, creed, or any other classification protected by federal or state law. Holon’s non-discrimination commitment is consistent with Section 1557 of the Affordable Care Act and applicable state law.
Language assistance services and auxiliary aids are available to you at no cost. To request a language interpreter, a sign language interpreter, or an alternative format (large print, audio, accessible electronic), contact us using the information at the end of this document.
Quality and qualified care
You have the right to receive timely care from qualified clinicians and to receive the least restrictive, most appropriate care available. Holon’s clinicians are licensed in the state where you are located at the time of your visit. Holon’s Peer Recovery Specialists are certified in each state where Holon operates.
Participation in your care
You have the right to participate in decisions about your care, including the development of your care plan. You have the right to express informed consent for the release of your information, and to refuse care or treatment to the extent permitted by law, having been informed of the consequences.
Information about your care
You have the right to receive, in language you understand, information about your diagnosis, treatment options, expected outcomes, and risks; about the providers who are treating you; and about Holon’s policies and procedures that affect your care.
Privacy and confidentiality
You have the right to privacy during clinical visits and communications. You have the right to confidentiality of your records, including the federal protections of HIPAA, the federal substance use disorder confidentiality rules at 42 C.F.R. Part 2, and applicable state law. Our Notice of Privacy Practices describes how we use and protect your protected health information and your specific HIPAA rights.
Access to your records
You have the right to inspect and obtain a copy of the protected health information Holon maintains about you, including in electronic form, and to direct us to send a copy to a person or entity you designate. You have the right to request an amendment to your records and to an accounting of certain disclosures.
Right to be notified of a breach
You have the right to be notified if a breach of your unsecured protected health information occurs.
Right to know about artificial intelligence
Holon uses automated tools and artificial intelligence (AI) to support your care — for example, to personalize reminders, to support your clinician’s documentation, to verify activities for the Rewards program, and for compliance and quality. You have the right to know that AI is being used in your care, and you have the right to request that a Holon team member review any communication you receive that was generated with AI assistance. Clinical decisions about your care are made by Holon clinicians, not by AI alone.
Good Faith Estimate
If you are uninsured or self-pay for any service, you have the right to receive a written Good Faith Estimate of expected charges before your scheduled service, consistent with the federal No Surprises Act.
Right to consult and to refuse
You have the right to consult with legal counsel and other clinicians of your choosing. You have the right to refuse treatment to the extent permitted by law.
Telehealth choice
You have the right to refuse telehealth at any time and to request an in-person visit, where available, without affecting your right to future care.
Complaints — without retaliation
You have the right to file a grievance or complaint with Holon, with the U.S. Department of Health and Human Services Office for Civil Rights, with your state Medicaid program, and with the appropriate state professional licensing or oversight body. Filing a complaint will not affect the care you receive from Holon.
End treatment and obtain records
You have the right to end treatment with Holon at any time and to request that your records be transferred to another provider.
A copy of your rights
You have the right to receive a copy of these rights, of the Notice of Privacy Practices, and of any consent or authorization you sign with Holon. The current Notice of Privacy Practices is available at holonhealth.com/notice-of-privacy-practices and from the Holon Privacy Officer.
